GLP-1 treatment alongside type 2 diabetes
These medicines began as diabetes treatments. Combining weight loss treatment with diabetes medicines takes coordination. This page explains how the two work together.
The overlap #
Tirzepatide and semaglutide are licensed for both type 2 diabetes (as Mounjaro and Ozempic) and weight management (Mounjaro and Wegovy). Many people starting weight treatment already have prediabetes or diabetes, and for them, weight loss and blood sugar improvement arrive together.
Medicine combinations needing care #

| Metformin | Usually continues unchanged. Combination is routine. |
| SGLT2 inhibitors (dapagliflozin etc.) | Usually compatible. Hydration matters more. |
| Sulfonylureas (gliclazide etc.) | Hypo risk when combined, your GP may reduce the dose before you start. |
| Insulin | Doses often need reduction as weight falls. Requires GP or diabetes team coordination. |
This is why we ask for your GP details. Joined-up care is not bureaucracy. Joined-up care prevents hypos.
Recognising low blood sugar #
If you take sulfonylureas or insulin, know the hypo signs: shaking, sweating, fast heartbeat, hunger, confusion. Treat with fast sugar (glucose tablets, juice), then recheck in 15 minutes. Tell us and your GP if hypos appear, doses need adjusting, which is a sign of success, not failure.
An eye note #
Rapid improvement in blood sugar can temporarily worsen diabetic retinopathy. If you have known retinopathy, keep your eye screening appointments current and report any vision change promptly.
Type 1 diabetes #
GLP-1 weight loss medicines are not licensed for type 1 diabetes and we do not prescribe them for this condition. Specialist teams sometimes use them experimentally alongside insulin. This decision belongs to your diabetes consultant.
