How to take the Wegovy pill
The Wegovy pill only works when you take the tablet in a set way. You need an empty stomach, a small amount of plain water and a 30 minute wait before food, drink or other medicines.
In short: Take one Wegovy tablet a day on an empty stomach after fasting for at least 8 hours. Swallow the tablet whole with a sip of plain water, up to half a glass (120 ml). Then wait at least 30 minutes before you eat, drink or take other oral medicines. A shorter wait reduces absorption. If you miss a tablet, skip the missed dose and take your normal dose the next day.
The morning routine, step by step #
Most people take the tablet straight after waking, because a night of sleep supplies the 8 hour fast.
- Check you have had no food for at least 8 hours.
- Take one tablet out of the blister with dry hands.
- Swallow the tablet whole with a sip of plain water. Use no more than half a glass, which is 120 ml.
- Start a 30 minute timer.
- During the 30 minutes, have no food, no drinks and no other oral medicines.
- After 30 minutes, eat breakfast and take your other medicines as normal.
Do not split, crush or chew the tablet. Take one tablet a day and never two.
Why absorption depends on the routine #
Semaglutide is a large molecule which the gut normally breaks down. Each Wegovy tablet contains an absorption helper called salcaprozate sodium, and the tablet is absorbed mainly in the stomach. Even with perfect technique, only about 1% to 2% of the semaglutide in a tablet reaches your blood.
With a margin this small, the conditions in your stomach decide whether a dose works. The product information says absorption drops when the tablet is taken with food or with large volumes of water. Longer fasting before and after the dose gives higher absorption. Waiting less than 30 minutes reduces absorption.
The product information also warns of variable absorption. If your weight loss is lower than expected, the first thing your prescriber will check is how you take the tablet.
Other medicines and the 30 minute rule #
Wait at least 30 minutes after your Wegovy tablet before you take any other oral medicine. Swallowing several tablets together reduces how much semaglutide you absorb.
Levothyroxine
Levothyroxine needs extra care. In a study, total thyroxine exposure rose by 33% when levothyroxine was taken with oral semaglutide. The product information advises monitoring of thyroid function, so tell your prescriber and your GP. Take the Wegovy tablet first and leave at least 30 minutes before levothyroxine. Ask your prescriber or pharmacist to confirm the order and timing for you, because levothyroxine has timing rules of the manufacturer's own.
Other medicines to mention
- Warfarin and similar blood thinners: your clotting may need checking more often when you start.
- Insulin or a sulfonylurea such as gliclazide: the risk of low blood sugar rises, and your diabetes dose may need lowering.
- The combined contraceptive pill: studies found no relevant change in hormone levels. Vomiting or severe diarrhoea can still affect any oral pill, so see pregnancy and contraception.
List every medicine and supplement you take in your online health check. People with diabetes can read GLP-1 medicines and diabetes.
Coffee, tea and breakfast #
Coffee and tea count as drinks, so both wait until the 30 minutes are up. Only plain water goes with the tablet, and only up to 120 ml.
Milk, juice and protein shakes also wait. A breakfast with protein helps you meet your daily target, and the protein guide gives amounts.
Shift work, late nights and travel #
The rule is an 8 hour fast, and the product information does not tie the dose to a clock time. If you work nights, take the tablet when you wake from your main sleep, provided your last food was at least 8 hours earlier. Then wait 30 minutes before eating.
A late meal or a snack at 2 am resets the 8 hours. On those days, delay the tablet until 8 hours have passed. If the day gets away from you, skip the dose and restart the next day. For flights and time zones, see travelling with GLP-1 treatment.
Missed tablets, extra tablets and storage #
If you forget a tablet, skip the missed dose and take your normal dose the next day. Never take two tablets to make up. The missed dose guide covers longer gaps.
If you take more tablets than you should, contact a doctor or NHS 111 straight away. You may feel sick.
- Keep tablets in the original blister until the moment you take one. The blister protects the tablet from moisture and light.
- Do not move tablets into a weekly pill organiser.
- The tablets need no special storage temperature and no fridge.
- Keep the pack out of the sight and reach of children.
Common mistakes which weaken the dose #
| Mistake | Why this matters | What to do instead |
|---|---|---|
| Taking the tablet with coffee or tea | Only plain water is allowed with the dose | Use up to 120 ml of plain water |
| Drinking a full glass of water | Large volumes of water reduce absorption | Take a sip, half a glass at most |
| Eating after 10 or 15 minutes | A wait under 30 minutes reduces absorption | Set a 30 minute timer |
| Taking the tablet after a late snack | Food in the stomach reduces absorption | Leave 8 hours after your last food |
| Swallowing other pills at the same time | Several tablets together reduce absorption | Take other medicines after 30 minutes |
| Crushing or splitting the tablet | The tablet must be swallowed whole | Swallow whole, one tablet a day |
| Doubling up after a missed day | Two tablets together are not allowed | Skip the missed dose, resume next day |
| Storing tablets in a pill organiser | Moisture and light can damage the tablet | Leave tablets in the blister |
If daily fasting rules will not fit your life, a weekly pen may suit you better. Compare the two on the Wegovy tablets vs pen page.
How to take the Wegovy pill: questions #
Can I take the Wegovy pill with coffee?
No. Take the tablet with plain water only, up to 120 ml. Coffee, tea, milk and juice all wait until at least 30 minutes after the tablet. Drinking anything other than a small amount of water with the dose, or too soon after, reduces how much semaglutide your stomach absorbs.
How long after the Wegovy pill can I eat?
Wait at least 30 minutes before you eat, drink or take other oral medicines. The product information says a wait shorter than 30 minutes reduces absorption, and longer fasting after the dose gives higher absorption. Set a timer each morning so you do not guess.
Can I take the Wegovy pill at night?
The required fasting window matters more than the clock time. The leaflet specifies at least eight hours without food before the tablet, then at least 30 minutes before food, drink or other oral medicines. Discuss a workable routine with your pharmacist.
What happens if I miss a Wegovy tablet?
Skip the missed dose and take your normal dose the next day. Do not take two tablets together. Semaglutide has a half-life of about one week, so a single missed day has a small effect. If you miss several days in a row, message your clinical team before restarting.
Do Wegovy tablets need to be kept in the fridge?
No. The tablets need no special storage temperature. Keep each tablet in the original blister pack until you take one, because the blister protects against moisture and light. Do not transfer tablets to a pill organiser, and keep the pack away from children.
